Business Support Application Form Description
This application form is designed to assess and support individuals and organizations engaged in various business sectors, particularly those from vulnerable communities, including internally displaced persons, refugees, young women living with HIV, orphans, and unemployed youth.
Applicants are required to provide their personal and business details, including demographic information, education level, business location, and sector. Key business sectors include formal and informal vocational trades, agriculture, food processing, beauty, wellness, ICT, and more.
The form also collects essential business insights such as the applicant’s background, organization profile, market opportunities, financing objectives, and operational model. This includes details on business activities, target markets, competitive positioning, and financial needs. Additionally, applicants must outline their growth potential, key strengths, supply chain, production processes, and job creation prospects.
Finally, references are requested to verify the applicant’s background and business credibility. Responses to application questions exceeding 500 words per section will not be considered.

Please download the form and send it via email to EEEPmicrocredit@ghsscm.org. The subject of the submission email should be the name of the region in which the project will be implemented and the applicant’s name. For example East Region – Mojoko Phillipe.
